Payments

We give you a Superbill invoice for you to send to your Insurance carrier, you must submit it with your CMS 1500 form. That has your personal information for insurance company.
Most Employers have a HSA spending program . You may submit your superbill or if you have HSA spending card.
We only process Veterans insurance with prescription and authorization.
We also process Landmark Coverage patients.
Medicare and Medicaid are not covered for visits.
